Medical Center "LIEPA" invites you to remember to bring the necessary documents with you when going to the doctor!
For adults, these are:
personal identification document - passport or identification card (a driver's license is not a personal identification document!);
insurance policy (if any);
results of recent examinations and tests, if available to you;
referral from a family doctor or specialist, if the payer is an insurance company.
Children under the age of 14 can only visit the doctor with an adult! If you visit the doctor with one of your parents, you must have:
the child's birth certificate, which indicates the parent's name and the parent's passport or identification card, or
the child's passport or identification card and the parent's passport with an entry about the child.
If a child visits a doctor with another adult (sister, brother, grandparents or other relatives), they must have:
a notarized power of attorney to represent the child's legal rights and interests;
adult's passport or identification card.
Children aged 15 and over must bring the following with them when visiting a doctor:
passport or identification card.
